Apply to become an owner, tenant or additional resident in a ProCare-managed community. Complete the application online — the same information as our paper forms, with no printing, scanning or mailing.

Application fee

$150.00

A non-refundable application fee accompanies each application. Married couples may submit a single joint application and fee. All other adults must submit separate applications and fees.

Before you start

The name of the association or community, and the property address.
Each applicant's current and previous address, Social Security number, date of birth and driver's license.
Closing date (purchase) or lease dates (tenant), when they apply.

How we protect your information

Social Security numbers, dates of birth and driver's license numbers travel over a secure connection and are encrypted with AES-256-GCM before they are stored. Only authorized ProCare staff can view them, and they never appear in emails or in the PDF copy of your application.
